Wheelchair Donations
by Buddha's Light International Association (Sec 21)
Wheelchairs, four-legged aluminium walkers & wooden crutches sponsored by Buddha's Light International Association (Sec 21) are donated from time-to-time for compassionate reasons to needy individuals or organizations, subject to returning by ordinary mail (not email or fax) - or delivering by hand - to us the completed & signed ORIGINALS of the following documents, and subject to our approval of your application:

Individual Applicants:

b) and attach the following:
Organizations only:
- Application Form From Organizations & Organization Recipient's List completed & signed by the recipient and/or contact person;
- Copy of each applicant's / recipient's ID-book photo-page - stamped & signed OK by his doctor, clinic or social worker; or a separate Letter from a Doctor or Clinic why the recipient/s need the item/s applied for;
- Your Organization's Prospectus, Pamphlet, Intro Letter & Copy of NGO or NPO form (preferably with some pictures of the premises or staff);
- Motivational Letter on your organization's letterhead, preferably written & signed by the head of the organization;
- BLIA Terms & Conditions Form signed & dated at the bottom of the page by the applicant;
- 「Thank you letter to BLIA」 (preferably handwritten), on delivery of the donated items, for example:
「I/we (your or your organization's name, surname, title, RSA ID No) would hereby like to thank the Buddha's Light International Association for the …… sponsored. I / we greatly appreciate this donation…..」
Individual Applicants:
- Application Form From Individuals completed & signed by the recipient and/or contact person;
- Copy of the applicant's / recipient's ID-book photo-page - stamped & signed OK by his doctor, clinic or social worker; or a separate Letter from a Doctor or Clinic why the recipient needs the item/s applied for;
- Motivational Letter, preferably hand-written & signed by the applicant;
- 1x Passport-size photo of the recipient attached to the Application, top right corner;
- 1x Full-body photo of the recipient attached, black & white or colour, any size;
- BLIA Terms & Conditions Form, signed & dated at the bottom of the page by the applicant;
- 「Thank you letter to BLIA」 (preferably handwritten), on delivery of the donated items, for example:
「I/we (your or your organization's name, surname, title, RSA ID No) would hereby like to thank the Buddha's Light International Association for the …… sponsored. I / we greatly appreciate this donation…..」
